AWARDS APPLICATION FORM

WARNING:

For some reason, the code for the form for this doesn't
work properly with all domains. The submit button will
cause your email program to bring up a new mail window.
Please carefully check the email window to make sure all
fields are filled out before you click on the send button.
If there are empty fields, please fill them out with the
information below.

1) The To: field should have the email address
[email protected] in it.

2) The subject line should say "Awards Request".

3) If only the body (the large field)of the email is blank,
highlight all lines beginning with a *, then hold down the Ctrl
& press the C key on your keyboard (at the same time)
to copy, go to the email & hold down the Ctrl & press the V key
on your keyboard to paste into it.

4) Now, just answer all the questions & click on the send
button. You will get an email back with my decision.
